When you submit the birth-plan intake form, you confirm that you have read and agree to the following. This helps us support you well and keeps our relationship clear and trustworthy.
Non-medical doula support
DouLaurin provides emotional, physical, and informational support before, during, and after birth. A doula does not provide medical care or advice, does not diagnose or treat any condition, does not perform clinical or medical tasks, and does not make decisions for you. Our support works alongside, and never replaces, your physician, midwife, or other licensed healthcare providers. Always consult your qualified provider for medical decisions.
Your preferences, your decisions
The information you share describes your own wishes and preferences for your birth plan. Any clinical or newborn-care choices are decisions to discuss and make with your medical provider. We record your preferences only so your wishes are understood and supported.
How your information is handled
You consent to DouLaurin collecting and using the personal and health-related information (PII/PHI) you provide to prepare your birth plan and provide support, as described in our Privacy Notice. Your information is stored in our secure client-management system and shared only with the service providers that help us operate.
No guarantee of outcomes
Every birth is unique. We cannot and do not guarantee any particular birth experience or outcome. Plans often change, and that is okay — our role is to support you with care and respect, whatever unfolds.
Voluntary & revocable
Sharing this information is voluntary, and you may ask us to update or delete it at any time by contacting us. Submitting the form places you under no obligation to book services.
